Behind Taroko tragedy and other construction accidents, a pattern of negligence

Taiwan is no stranger to deadly construction accidents. In 2021, a construction vehicle caused an express train to derail, claiming 49 lives and injuring more than 200 people. In April 2015, a steel girder fell over the worksite of the Taichung metro’s Green Line, taking the lives of four people and injuring another four. Experts say these accidents are part of a widespread pattern of negligence.

On the first day of the 2021 Tomb-Sweeping Festival holiday, a construction vehicle fell onto a section of train track in Hualien County. A high-speed Taroko Express train slammed into it, resulting in 49 deaths.

The train had been running on the railway’s Eastern Trunk Line, but the problem lay along the Western Trunk Line, where a tunnel slope maintenance project was underway.

Fingers were pointed all around in the wake of the accident. According to risk management expert Daniel Yao, the construction site was managed by three companies. Not one of them realized that the site was full of risks.

This construction project was taking place right above an active railway, so that’s the first risk. The second is that there are two tunnels, meaning that if anything falls onto the tracks, the driver doesn’t have much time to react.

Slope maintenance on the open-cut tunnel began nearly two years prior. Workers from the three companies came and went, oblivious to the life-endangering risks of their project.

This accident occurred because a construction vehicle had slipped down a construction access road, it slipped down a slope. From the original worksite planning team, all the way down the line, no one saw this risk.

Construction accidents like this occur with an unsettling regularity.

On the afternoon of April 10, 2015, a 209-ton steel girder fell over a Taichung Metro construction site, killing four and leaving four injured. An investigation found that the contractor, Far Eastern Construction, had violated work hour regulations and flouted proper lifting procedures, leading to the tragic accident.

Nine months earlier, there were already signs of problems with the construction company.

In August 2014, Far Eastern Construction had improperly handled a crane, causing it to tip and crash into a residential building. The accident happened late at night, and no one was injured. In the months afterward, neither the contractor nor the government took steps to address the incident, which captured little public attention. A major accident struck the following year.

回顧這幾年,國內重大工安事故頻傳,包括在2021年清明連假期間,台鐵太魯閣號不幸發生出軌意外,造成49人死亡、200多人受傷的嚴重事故。2015年4月,當時施工中的台中捷運綠線也曾發生鋼樑掉落,8人死傷的事故;但這些意外的發生,真的都是「意料之外」嗎?

2021年清明連假第一天,一輛工程車掉落鐵軌,高速行駛的太魯閣列車猛然撞上,造成49人死亡。

造成這起事故的關鍵,並非太魯閣列車本身運行的東正線,而是另一端,正在進行隧道邊坡維護工程的西正線工地。

事故發生後,各界開始究責。但風險管理專家姚大鈞反而在意,明明這塊工地由三家公司共同管理,為何沒有一家公司事先察覺,工地滿是風險。

明隧道邊坡維護工程,開工近兩年,三家公司不同人員,在此進進出出,卻沒有人認知到,自己正在施作一項隨時可能危害鐵道安全的高風險工程。

遺憾的是,這樣的工安意外,幾乎每隔一段時間,就會傳出。

2015年4月10日下午,台中捷運綠線工地一根重達209噸的鋼梁突然重重摔落,造成4人死亡、4人受傷,究其原因是承包商遠揚營造,違規在白天施作,加上吊裝作業,沒有依照正常程序施作,才不幸釀成意外。

而這早在九個月前,遠揚營造一次意外事故中,就露出端倪。

2014年8月,遠揚營造因為不當操作吊車,使得吊車失去平衡砸向一般民宅,不過當時意外發生在深夜,加上沒有人員傷亡,事件過後,包商、政府,都沒有積極處理,社會也不重視,結果隔年就發生重大意外。
