
Although the National Health Insurance Administration has started promoting at-home dialysis, only 10% of Taiwan’s 90,000-plus people on dialysis are taking advantage. According to Linkou Chang Gung Memorial Hospital, at-home dialysis treatment can reduce the risk of cardiovascular-related hospitalization and death in elderly patients by 30%. A 90-year-old patient shared his experience, saying that at-home dialysis has simple instructions and is very manageable.
This old man was smiling from ear to ear as he celebrated his 93rd birthday with familiar medical staff. What left him nearly speechless was that he no longer needs to go to the hospital for dialysis.

We have an automated peritoneal dialysis system with a cloud monitoring function, so that we can monitor the situation remotely and if anything happens, our nurses can intervene. We have also developed an innovative assistive device that can help elderly people with poor vision or unsteady hands.
Over 90,000 people in Taiwan are on dialysis. More than 90% receive hemodialysis in hospitals, while only 6.7% use at-home peritoneal dialysis. At Linkou Chang Gung Memorial Hospital, one-quarter of patients on peritoneal dialysis are age 65 and over. Doctors have confirmed that compared to hemodialysis, seniors using at-home peritoneal dialysis have a 30% lower chance of cardiovascular issues, hospitalization, and death. At-home dialysis is not a privilege reserved only for younger patients.

The National Health Insurance Administration has invested NT$430 million in promoting at-home dialysis, helping hospitals to open up more beds. Doctors have also called for more diverse dialysis treatments as Taiwan faces the challenges of a super-aged society.

于爺爺的太太也已年過八旬,兩人互相照顧,是醫療團隊口中的模範夫妻檔,也被譽為是推動高齡腹膜透析的最佳示範。

"我們現在有一種自動腹膜透析,搭配雲端監控系統,遠距監控,所以有任何的狀況的話,我們護理師都會主動的介入。另外我們也有創新研究了一個輔具,可以幫助視力比較不好,或者手比較不穩的這些長者。"
全台洗腎人口超過9萬人,超過9成都在醫院接受血液透析方式來洗腎,能夠居家執行的腹膜透析卻只占6.7%,而林口長庚卻有4分之1腹膜透析病友,是65歲以上長者,醫師證實腹膜透析洗腎的長者,比起血液透析,罹患心血管疾病、住院風險和死亡風險都會下降3成左右,因此居家洗腎其實並非年輕病友特權。

"做腹膜透析不只是,你在家裡非常的方便,因為那就是你自己的居家環境,你不用到醫院來,而且經過這一波疫情,其實大家也相信,在家裡一定會比在機構安全。在政策方向上其實絕對是,堅持要繼續鼓勵腹膜透析的擴張。"
健保署投入4.3億元推居家洗腎,讓醫療院所也不必因此再開病床,醫師也呼籲面對超高齡社會考驗,未來長者洗腎治療規劃,可以更多元。
