Long-term care of pets becomes central as ownership increases

In Taiwan, the number of pet cats and dogs have surpassed the number of children aged 14 and under. Instead of having children, more and more people are keeping pets, treating them like their own furbabies. But even though pets are dependent on their owners their whole life, they won’t stay as “babies” forever. Like humans, they could get chronic diseases in old age and require frequent medical treatment. Of course, there are owners that choose to give up their aging pets, but what should responsible owners be prepared for? Let’s find out more in our special report.

This is the long-term care center at New Taipei Zhonghe Animal Shelter. All the dogs here are over the age of 12, also known as geriatric dogs. Even though it’s an open space with plenty of room for them to move freely, many of them look unsteady on their feet, and some can only lie down.

Humans age, and so do other animals, at a faster pace. Dogs have a higher metabolic rate than humans, and they also age five to seven times faster. At 7, they’re considered senior already. Age 14 for a dog is around 88 for humans.

Furball the Pomeranian here is already 18 years old, 90 in human years. It’s given medication for heart disease. Pets are living longer than ever thanks to advancements in animal healthcare. But on the other hand, living longer also means they could develop joint problems and other chronic diseases.

A common problem is chronic kidney disease. Most people are also aware that joints degenerate because of wear and tear with each use. It’s inevitable. There’s also nerve problems, as well as heart, intestinal and liver problems. In older dogs, they may eat less and less, and you may notice that their weight is constantly decreasing.

The fridge is stocked with more food for dogs than humans. Ho Tsung-hsun, secretary-general of Taiwan Animal Protection Monitor Network owns Chi-hsin the Husky with his flatmate. For the past nine years, he’s taken care of her like his own child, preparing three meals and snacks every single day.

I went through many trials and errors to get to this meal plan, which is also the healthiest. Many rescuers suggest having three staple vegetables: cabbage, broccoli and carrot. This prevents cancer and is the healthiest, including for humans.

Ho’s learned to make meals for his little one. But no matter how nutritious, Chi-hsin could get tired of the same meals over and over, so there’s also a range of seasonings.

You have to make the meals feel different, so I have all kinds of seasonings. Even humans can’t stand having tofu every day. Wouldn’t you also get tired of having biscuits every day? It’s the same with animals. There has to be variety. You can’t only have this. Of course, she may feel differently, and it may be human projection.

As a senior dog, Chi-hsin is starting to show signs of aging. She’s no longer as lively and also ambivalent about playing. She’s also diagnosed with cholecystitis, requiring regular follow-ups with the vet. However, pets aren’t covered by universal healthcare, making each visit very costly.

Even though it’s only been a little less than two months, I’ve already spent around NT$15,000, really a lot of money. For a human, three visits to the doctor is only NT$600, so I really didn’t expect to spend so much. After every visit, the vet asks if you’d like to follow-up, which would cost another 2,000 to 3,000. But do you really want to risk it worsening? So it’s an expense that you can’t cut.

Ho may spend a lot of money, but he’s also lucky enough to work from home and be with his dog. Most office workers can’t. On top of the expenses, they may also feel like they can’t spend enough time with aging pets.

Momo the Shiba Inu is 16 years old. After it developed heart disease, the owner no longer had the capacity to take care of it, so it’s put under long-term care at the animal hospital.

In 2024, the number of dogs and cats over the age of 7 accounted for 47% of the total. The market for older pets is estimated to worth at least NT$15 billion. Some animal hospitals in Taipei offer day-care. Owners can drop off their pets before the workday and fetch them after work. That costs NT$600 to NT$900 a day. There’s also long-term care, at NT$1,200 to 1,600 a day. Treatment and meals are billed separately.

It’s the same with older adults admitted to nursing homes. Many older adults worry that their kids are abandoning them when they’re admitted. But that’s not necessarily the case. Adult children have their own lives and may also be busy with work. It’s the same with animals. You can’t possibly take care of them all day long. You need to help them change positions, feed them, clean up after them. But if we have neither the time nor capacity, the animals may actually be worse off if continued to be kept at home.

Some owners are willing to shell out for day care and long-term care. But not all older pets are so lucky. Many rescued dogs at the animal shelters are so old that they could no longer stand. They were most likely abandoned.

It must’ve had an owner before considering how old it is. Strays don’t usually live to become so old. Most strays won’t survive past the age of 10, because they easily get into accidents. There could be cars or other injurious scenarios in the wild.

With limited staff, the shelter residents can’t really be taken care of one-on-one. And so members of the public are encouraged to adopt older pets.

Every single one of them need to take painkillers and joint supplements, requiring frequent medical intervention. It really adds to the burden of frontline workers, whether physically or psychologically. An animal protection officer may have just put down pee pads and soon they’ll get soiled, and then there’s cleaning up to do. It really makes the job complicated for them.

Chu Chan-sheng has treats ready for geriatric dogs at the shelter. He can’t bear seeing them living their last days without their owners, so he tries to adopt as many as he can.

We had older dogs at home before. After many years, they became old and ran into medical problems. This all became lived experience. We’d take the vet’s advice and learn how to take care of them and what their treatment requires.

As a real estate agent, Chu’s schedule is quite flexible. He had nine dogs at one point. But how did he plan for them financially?

Vets charge based on their weight. We had a dog that weighed more than 30kg. The medical costs until it passed away was at least NT$400,000. My partner and I are frugal. We try to be as frugal as we can on our own expenses. It’s okay if we don’t get to have as many luxuries, but we can’t cut their medical expenses. We try to save with our own expenses.

Chu currently has adopted two geriatric dogs and one cat. One of the dogs has dementia and has to be tube fed.

It doesn’t know where food is, or where water is, so we have to tube-feed it. It also keeps walking. If you don’t make it rest, it’ll just keep going and going. It wouldn’t know when to stop and rest.

Not all owners are so patient with aging and sick pets. Some may even take out anger on them.

Recently, a 15-year-old Corgi with dementia was filmed while beaten by its owner because it had releved itself where it shouldn’t. The owner posted the video online, hoping to gain sympathy. Instead, he was widely condemned.

When pets have this kind of behavior, it’s really not out of their own will. It’s just a natural part of aging. The owner taking his anger out on the dog doesn’t solve the problems. I think it’d be better for him to seek advice from other owners of dogs with dementia. That way, I believe he’d have it easier.

Even though the animal shelter encourages adoption, few adopters ever show up. So the geriatric dogs have to make do with government funding until death. If not for incurable diseases or loss of mobility, and if they still have normal appetite and energy levels, the animal shelter won’t consider putting any of them down.

As for pet owners, they can consult a vet if they’re considering euthanasia.

Let’s say that the future you foresee is just worse and worse by the day. If the quality of life is so low that it’s stopped eating and could only be tube fed, or if it’s lost all function, or if you’re noticing that the pet is unhappy or suffering every single day, then I’d suggest that the owner let them become an angel.

In the end, it’s still up to the owner. But there will be a point when the medical bills just get too overwhelming and you’ll have to make up your mind.

People may want pets for amusement or companionship. But no matter the reason for getting a pet, it’ll eventually become old and frail. Would you stand by it until the last moment, or leave it to an animal shelter? Do you know what to expect for a pet’s future?

寵物高齡化時代來臨 長期照護成新興市場 醫療費用與棄養問題浮現

現代人養寵物,大多會把牠們、當成家人一樣呵護。毛小孩的壽命不比人類,飼養年看體型,小型犬最多可養到18年,只不過,毛小孩也跟人類一樣會生病,也會有慢性病,醫療費相當可觀。有的飼主會選擇照顧到終老,但也有人選擇遺棄,如何好好照顧毛小孩,飼主又該有哪些心理準備?請看我們的專題報導。

走進新北市中和動物之家長照園區,眼前的狗狗年齡都超過12歲,屬於高齡犬。雖然是開放式空間,讓牠們自由行走,但很多隻看上去步伐不穩,有的甚至只能趴著。

人會老,動物也會老,以狗為例,因為新陳代謝比人快約1.5倍,老化速度也比人快,一般而言,狗的老化速度是人類的5到7倍,7歲以上就被視為熟齡和高齡寵物,像是狗14歲相當於人類88歲。

這隻博美狗Furball已經18歲,相當於人類90歲,吃的是心臟病的藥。由於醫療越進步,寵物越長壽,不過年紀大,多會伴隨關節以及慢性疾病。

“比較容易出現慢性腎臟病,大家可能比較知道的,骨關節也是因為退化,每次在使用中,所以關節一定會磨損,再來有些是神經的問題,再來有些當然其他心臟
腸道、肝臟問題也都會遇到,在老狗可能會遇到的是牠們在吃的部分越吃越少,你可以發現牠們好像體重一直在下降。”

冰箱裡狗吃的東西比人的還要多,在台灣動物保護行政監督聯盟,擔任祕書長的何宗勳,與室友共同飼養一隻哈士奇-吉心,九年來每一天就像照顧自己的小孩一樣,為牠準備三餐和點心。

“我花了很長的時間,目前這種吃法是最健康,很多的愛爸愛媽,牠們的建議就是固定三種蔬菜: 高麗菜、花椰菜跟紅蘿蔔,又可以抗癌,是最養生的,包括人類。”

為愛犬學做菜,雖給牠吃得很營養,但又怕牠吃膩,所以還特別準備各種調味料。

“你為了讓牠感覺有變化,所以我就有各種不同的調味粉。人類的話,怎麼可能每天都吃豆干?如果每天要吃餅乾,你不會膩嗎?所以動物也是,每天有變化,你不能只有吃這個,當然牠是不會有什麼差別的,但因為這是人類的投射。”

不過,進入高齡的吉心,已出現典型老化現象,除了不比從前活潑,跟牠玩也愛理不理,而且吉心又檢查出患有膽囊炎,需要定期回診,因為寵物沒有健保,出入一趟診所,醫療費很可觀。

“才看不到兩個月,我已經花了一萬五左右,很燒錢,如果我是人類的話,三次頂多六百元,所以沒有想到那麼多。看完了之後,他就問你要不要再複檢,我們想複檢就要燒兩、三千元,不複檢萬一惡化怎麼辦?所以錢你都不能省。”

由於何宗動可以選擇在家上班,同時陪伴愛犬,但對大多數有固定工時的上班族來說,尤其在照顧高齡犬上,不只花錢,也常覺得沒有時間照顧和陪伴。

Momo是一隻16歲的柴犬,因為有心臟病,家人照顧不來,所以帶來動物醫院做長期安養。

2024年有47%的犬貓超過7歲,銀髮寵物市場估計至少有150億元以上。在台北市,有動物醫院提供日托服務,飼主一早送過去,下班後再帶回家,一天費用從600到900元,另外也有提供安養服務,一天1200到1600,這些還不包括醫療和餵食。

“就跟人的安養機構觀念一樣,很多長者也會擔心說,我被子女帶去安養,是不是要被拋棄?其實不一定,因為小孩子他們長大也有自己的人生,所以他們工作也很忙,動物也是,你不可能每天都在照顧動物,牠可能一天要翻身好幾次,牠可能需要餵飯,牠可能需要把屎把尿,如果我們沒有這樣子的時間跟這樣的能力的時候,動物是不是在你家,反而會越照顧越不好。”

有的寵物有飼主花錢,幫忙住進日托和安養機構,但有的則是晚景淒涼。來到動物之家,這些被救援的狗,有些已經老到站不起來,而牠們很有可能是被棄養的。

“會到這麼老一定是有人養的,一般遊蕩犬不會那麼老還存在。遊蕩犬整個生存年不會到10歲以上,因為牠們在外面會有很多意外,像可能有一些車輛,或是在野外都會有一些,不小心造成牠們受傷的情況。”

人力有限,很難達到一對一照顧,動物之家因此開放民眾,前來認養這些高齡動物。

“每隻動物都需要餵一些止痛藥,或者是關節保護劑,所以牠的醫療處置會更頻繁,會造成第一線的工作人員,不管是體力或者是心理上其實負擔會更大。看動保員照顧,有時候剛放下去他清完的墊子,馬上就大便了,他變成又要再清一次,所以對他們來講,照顧上面會變得非常複雜。”

拿著零食到長照園區,探望高齡犬,他是朱展昇,捨不得狗狗在沒有主人的陪伴下孤獨死,只要有能力,就會來動物之家認養高齡犬。

“我們以前的狗,因為帶回家照顧之後,經過好幾年之後,他們也慢慢有年紀了,有年紀的時候,我們會遇到一些醫療上的問題,這些都是吸取的經驗,譬如說照顧,我們都是聽取醫生的建議,照顧的方式,醫療的方向怎麼做。”

朱展昇本身做房仲業務,工時很彈性,曾最多一次領養9隻狗,不過高齡犬免不了開銷大,怎麼面對龐大開銷呢?

“獸醫都是看牠的體重,我們曾經有一隻狗大概30多公斤,牠的醫療費用到整個牠離開,大概花了最少有40萬。我跟女朋友比較節省,就是自己的開銷,自己過不好一點沒關係,但牠們的醫療沒辦法省,人能省的地方都盡量省。”

目前朱展昇認養2隻高齡犬 1隻貓,其中一隻狗還罹患失智,必須要灌食。

“牠也不知道吃飯的地方在哪裡,水在哪裡牠也不是很清楚,我們就要灌食,到現在牠會一直走,你沒有強制讓牠休息,牠就一直走,走到牠不知道什麼時候要休息。”

長期照顧高齡又生病的寵物,有些飼主受不了壓力,情緒失控。

日前一隻15歲罹患失智的柯基犬,被飼主打罵,原因是亂大小便,飼主氣得將影片Po上網,原本想凸顯照顧老狗的不易,沒想到卻得到反效果,遭到撻伐。

“寵物牠們會有這樣的行為,不是牠們的意願,那是自然的老化,所以飼主這樣的反應,發洩在狗的身上,對於問題是沒有辦法解決的。我覺得可以多聽一些,也在照顧這些失智犬的飼主心得,我覺得可以走一些,比較不會走到的冤枉路。”

雖然動物之家有開放認養制度,但願意認養的人很少,所以這裡的高齡犬,只能花國家公帑照顧到死亡,除非患有絕症或無法行走,即便是年邁,只要食慾精神還正常,動物之家不會考慮安樂死。

而一般飼主是否能對寵物進行安樂死,也可以找獸醫師討論。

牠未來的每一天,只會越來越不好,牠的生活品質是差的,牠已經不吃了,只能靠灌食,牠已經站不起來了,或者牠已經是沒有任何能力,你會發現這個動物每一天是在不快樂、或者是痛苦當中的時候,我會主動建議飼主,我們讓牠當小天使。

不過執行安樂死還是要看飼主的決定,獸醫師也建議,要設財務停損點,否則醫療費會是個無底洞。

有人當初因為牠的可愛,有人因為想排解寂寞,不管什麼原因養了牠當毛小孩,當牠們年華老去,你會選擇陪伴牠們走完最後一程,還是遺棄,留給政府解決?在寵物高齡化的議題之中,人類要學習到什麼功課?
